Just a reminder that the Chicago Trip permission/medical authorization form and instrument form are due on Monday, March 26.
The medication form and regulation contract agreement are due the morning of the trip, Wednesday, April 4.
You only need to turn in the medication form if you are providing any medications (prescription or over-the-counter) for your child.
The Conn-Selmer Factory Tour Release must be signed and returned on Monday, March 26, 2018.
Also, as Mr. Kalish mentioned at the trip meeting, and as is mentioned in the Release, Open-toed and open-heeled shoes of any type are NOT allowed to be worn during the tour.
